Do you have an accepted proof of rabies vaccine?

Proof of Rabies Vaccine to be uploaded (PDF, Word or JPG). Documents are reviewed. Dog licenses will not be issued without accepeted proof of rabies vaccine.

  • ACCEPTED examples of Proof of Rabies Vaccine are as follows:
    • Rabies Certificate issued by a Veterinarian.
    • Vaccination Certificate issued by a Veterinarian that includes rabies vaccination details, including next due date for the rabies vaccine.
    • Receipt from a veterinarian with rabies vaccination ONLY IF it includes the animal's information and the next due date for the rabies vaccine.
  • NOT ACCEPTED
    • Photo of the rabies tag
    • Prior year's licensing document
    • Expired rabies certificates
    • Incomplete rabies certificates
  • Distemper information and proof is not required and do NOT need be uploaded.

Optional - Your dog's microchip number and microchip issuer (Home Again, 24 Petwatch, Avid, etc) for free microchip registration in the Maine Dog Licensing Database and with 24 PetWatch. Please note you will receive email from 24 PetWatch following registration, but no further action is required.
